Post-streptococcal Myalgia and Protracted Febrile Myalgia syndrome – similar yet different

We compare cases of familial Mediterranean fever-related Protracted Febrile Myalgia (PFMS) and Post-Streptococcal Myalgia (PSM), both rare disorders presenting with fever, myalgia, and inflammatory biomarkers. Although clinical symptoms may be undistinguishable, steroids are usually required in PFMS and PSM most often respond to non-steroidal anti-inflammatory drugs. Awareness of PSM and preceding history may prevent unnecessary tests or overtreatment.
