What are brown dots in the pancake mix?

Three sisters with a history of atopic disease developed wheals, throat discomfort, and dyspnea about 30 minutes after eating okonomiyaki (Japanese savory pancake). The 11- and 13-year-old sisters were judged to have moderate to severe anaphylaxis and treated with intramuscular epinephrine. The symptoms in the 8-year-old sister were mild and disappeared after receiving an antihistamine.
